You really don't want to wear it inside-out.

The green slayer helmet is a piece of equipment that is worn in the helmet slot. It is a cosmetic upgrade to the slayer helmet.

It can be created by adding an unstuffed Kq head after purchasing the ability Kalphite khat for 1,000 Slayer reward points from any Slayer master. This is reversible, and the head will be returned to the inventory along with the slayer helmet's components. The Kq head will be lost on death in PVP.
