It can be created by adding an unstuffed Kq head after purchasing the ability Kalphite khat for 1,000 Slayer reward points from any Slayer master. This is reversible, and the head will be returned to the inventory along with the slayer helmet's components. The Kq head will be lost on death in PVP.
