A Pirate's hat is a possible reward from doing level 3 Treasure Trails, and can also be traded with other players. The pirate's hat can be stored in a player-owned house costume room.
Players who have completed Cabin Fever, and have a book o' piracy in their inventory, can pay Patchy 500 coins to attach a pirate's hat to an eye patch to create a hat eyepatch.
